MoHMIS User Access Request

This form may be completed solely by a designated authorized representative of the agency. Once this form is submitted, an ICA representative will review the request. The request will be reviewed:

  • to ensure it has been submitted by a designated authorized representative,
  • to ensure that the form appears to have been properly completed,
  • to determine whether an HMIS license is available (if an additional user is being requested), and
  • to ensure any other requirements or expectations that may exist have been met.

Submission of this form does not guarantee access to the MoHMIS will be granted, and anyone who will have access to the system must complete the proper training prior to being granted access. Information about the training process is available in the HMIS User Access Policies and Procedures guide. If you have any questions, please contact the helpdesk before proceeding.

Documentation Requirements

In the project selection area above, you indicated that this user needs to have access to the BoS Coordinated Entry system within ServicePoint. Per rules set forward by the Balance of State Coordinated Entry committee, for a user to have access to a MO BoS CoC CES Access Point, the current CES Memorandum of Understanding (MoU) for your agency and the user's BoS CES Training Certificate must be uploaded. BusinessObjects licensure

BusinessObjects Need-to-Know

A BusinessObjects License allows users to run "advanced" reports in the HMIS. The majority of reports within the HMIS are "advanced," and any users who will be responsible for any of the following will likely need a BusinessObjects license:

  • Running billing reports for submission to funders
  • Generating reports related to coordinated entry (i.e., prioritization lists)
  • Monitoring data quality and completeness for programs
  • Monitoring program performance

If you're unsure whether a user may need a BusinessObjects license, feel free to contact the helpdesk for guidance. Authorized representatives can also always contact the helpdesk via email at a later date to request a user be granted a BusinessObjects license.

SPDAT access

SPDAT Need-to-Know

The full SPDAT is currently available only within the St. Louis City and St. Louis County Continuums of Care. Users who do not have access to at least one project within the City or County CoCs may not have access to the tool in ServicePoint. Any questions about the rules regarding full SPDAT access in ServicePoint can be directed to our helpdesk.

The Service Prioritization Decision Assistance Tool (aka: the "full SPDAT") was developed by OrgCode Consulting, Inc. who mandates that any individual who will be administering the full SPDAT have completed proper, up-to-date training by OrgCode or an OrgCode certified trainer. The ability to access the full SPDAT in ServicePoint does not require this specific training, however all users who have access to the full SPDAT in ServicePoint must comply with the training requirements. Any users who have not completed the proper, up-to-date training may have access solely for the purpose of recording SPDATs administered by properly trained individuals and/or reporting on SPDATs that have been entered into the system, and must understand that having access to the full SPDAT in ServicePoint alone does not grant them the ability to administer the tool.
